Elders Choice FI LLC has been approved as a Fiscal Intermediary (FI) by the New York State Department of Health to provide CDPAP (Consumer Directed Personal Assistance Program) services.
Consumer Directed Personal Assistance Program (CDPAP) IS A Medicaid-funded program that allows New Yorkers to hire their caregivers: A daughter, son, relative, friend, or previously hired aide.

Am I Eligible
To qualify for CDPAP a person must:
- Be eligible for Medicaid
- Need help with daily activities
- Be able to direct their own care
- Choose their caregiver

A nurse will visit you at home to determine your eligibility for the CDPAP program. You'll find out how many weekly hours of care you can get.
Choose your caregiver
You choose a caregiver: your daughter, son, relative, or friend. We come to your home to enroll you in the program.
You get care — they get paid
You get ongoing care from a person who loves you. They get peace of mind — because they’re getting paid for the care they give.
